Application Process

This process is first come, first serve as there is limited space. 

A wait list will be available if we exceed our availability.

 

Please fill out the below application to start the vetting process.

We will contact you for the following information

after the application has been submitted:

  • Picture or copy of DD214, Military ID or Veteran ID 

  • Last four numbers of Social Security number 

  • Form of payment made out to VFW

and mailed to:

2024 Applications CLOSED

Applications received after 4/1/2024 will be for 2025 Banner program.

GUIDELINES FOR APPLICATION:

The individual must have served in any of the following branches of the US Armed forces

(Army, Marine Corp, Navy, Air Force, Coast Guard, Space Force, National Guard or Reserves)

and must also meet one of the following conditions

  • Currently be active service member or

  • Be an honorably discharged veteran.

and

Individual must live or have lived in Shawano (54166 zip code)

 

Nominee or next of kin must approve finalized banner and will receive banner after Veteran's Day.
